About this form

FRC Team 271 and Bay Shore STEAM Corporation (BS-STEAM), the nonprofit that supports the team, are asking former team members what they did after high school. Your answers help show students, families and sponsors where the team can lead, and the statistics count how many alumni stay involved in FIRST (no names).

Who keeps your answers: Bay Shore STEAM Corporation (BS-STEAM). Rich Varone and Bill Claiborne, the team's Lead Mentors, read them for BS-STEAM. They are stored with Cloudflare, which runs this form. This is BS-STEAM's form; the Bay Shore Union Free School District is not part of it.
How they are used: only as you allow in the three permission boxes at the end. Without the first box, nothing you send is used.

FIRST advises alumni to wait four years after graduation before mentoring their former team, and every mentor needs YPP clearance.

7 Permissions

Each box is separate. Tick only the ones you agree to. To change a box or remove your answers, write to Info@BS-STEAM.com with your reference number. BS-STEAM stops new uses and removes your answers from its websites and social media within 10 business days. Anything already printed, or shared by others, cannot be recalled. BS-STEAM keeps your answers until you ask for removal or it stops collecting alumni outcomes.

(a) You may use my answers in combined statistics with no names - including totals such as scholarship amounts and the kinds of employers alumni work for. (required to send) (b) You may name me with my outcome (class year, school or program, major, job title, and how I stay involved in FIRST) in team and BS-STEAM presentations, websites, social media, and sponsor and grant materials.
